What You'll Need To Do
- Call for an appointment (Indianapolis 317-338-2268 and Carmel 317-582-7158)
- Obtain two tuberculosis tests (we provide at no charge)
- Sign a confidentiality statement
- Obtain parental consent if you are under 18
- Have a blood screening (we provide at no charge)
- Consent to a criminal background check
- Attend a training session
