Medical Records Request
As a patient, you have the right to access your medical records. Here you'll find instructions and a convenient form to help us process your request.
St. Vincent retains hospital-based medical records in accordance with Federal and Indiana laws and regulations. Information contained in the patient’s medical record is confidential. For this reason, all requests for medical records must include a signed authorization by the patient (see the Medical Records Authorization form under locations).
St. Vincent HIM – Medical Records Department securely maintains electronic copies of our St. Vincent patients’ medical records as they relate to inpatient and outpatient services for hospital, outlying surgery center procedures, radiology and therapy services.
A fee is charged to cover the processing of requests. However, a fee is not charged when the copies requested are to be sent to a physician, as those requests are a part of your continuing care.

Monday - Friday during daytime hours at St. Vincent Indianapolis, campus at 8335 Naab Rd, Entrance #7., Indianapolis, IN 46260.
The Health Information Management (HIM) Department is located in the basement of the Heart Center/Cardiology Building. Park in the underground parking garage, B1 level, and take the Heart Center entrance at B1 (not Neuroscience Institute), through the double doors for a short walk to the Health Information Management Department.
