Community Health Needs Assessment for Sacred Heart Health System

A Comprehensive Community Health Needs Assessment (CHNA) provides the general public and policy leaders with data and information on the local health status of the communities we serve and existing resources to address health issues. The following documents summarize the most recent CHNA reports conducted with collaborating partner(s) as listed.

The CHNA uses a community-driven approach that supports the hospital in planning and carrying out community-based programming to more effectively handle the health needs of underserved and vulnerable populations.

The in-depth reports will help the hospitals and communities make decisions about the future of healthcare and prevention in their areas by identifying how their communities compare to the state on many dimensions of health. The findings point out several major health issues facing our communities, including levels of obesity, tobacco and substance use/abuse, access to health care and mental health services, and chronic disease care (e.g. diabetes, heart disease and respiratory disease).

The CHNA reports for each hospital are available at no expense. To save files to your computer, right click the file name and choose "save target as" or "save link as". You may also print a hard copy. Paper copies of the report are also available at the Hospital upon request. Requests should be made through the Legal Department by calling 850-416-6103.
