MyGULFCare: Helping you take control of your chronic condition
The MyGulfCare program is designed to help you manage your chronic health condition by providing personalized education and assistance to ensure the highest possible quality of life.
If you have one of the following health conditions:
- Diabetes
- Heart failure
- Chronic obstructive pulmonary disease
- High blood pressure
- High cholesterol
We are here to help!
As a MyGulfCare participant, you will have access to a care manager. Our care managers are Licensed Registered Nurses that are here to discuss with you your health conditions and assist you in chronic disease self-management. Our goal is to assist you in setting and meeting goals that will improve your health and your health outcomes.
Services we provide:
- Care management
- Help with social barriers
- Medication assistance
- Assistance in finding a doctor or specialist
Eligibility
MyGulfCare services are available to residents of Gulf and surrounding counties for people living with COPD, Heart Failure, Diabetes, High blood Pressure or High Cholesterol.

If you are interested in the MyGulfCare program, please complete the form below and have a front office staff fax it to: 850-229-5612. You can also drop off your form at the Welcome Desk of Sacred Heart Hospital on the Gulf.
