MyGULFCare is a program that helps low income residents of Gulf County access healthcare services. Offered by Sacred Heart Hospital on the Gulf in partnership with the Florida Department of Health in Gulf County, MyGulfCare strives to improve the health of Gulf County residents by coordinating care between primary care physicians, the emergency department, the department of health, and specialty care providers.

A variety of health services are provided at no or reduced cost to Gulf County residents who are uninsured or covered by Medicaid and who meet income eligibility criteria. 

Services available through MyGulfCare include:

  • Referrals to community services such as food or clothing assistance 
  • Appointments with primary care physicians for follow-up after visiting the emergency department 
  • Appointments with specialists 
  • Help with applying for financial assistance with hospital charges 
  • Help obtaining medications prescribed by a physician 
  • One-on-one education and training on how to self-manage diabetes, high blood pressure, or high cholesterol 
  • Personal and group coaching on losing weight, increasing physical activity, and achieving better health

Do you qualify for MyGulfCare?

To qualify for MuGulfCare’s free  or reduced cost services, you must meet the following qualifications:

  • U.S. citizen or legal resident 
  • Gulf County resident
  • Have Medicaid insurance or are uninsured
  • Low income

MyGULFCare - Pharmacy Assistance Program

If you cannot afford medicines prescribed by your physician, MyGulfCare may be able to help. Many medications for diabetes, high blood pressure, high cholesterol, asthma, and other chronic diseases are available from pharmaceutical companies for free or very low cost to low income patients. Our Pharmacy Assistance Coordinator can help you apply for this benefit. Proof of income, such as your latest tax return, and a prescription from your physician are required. This program does not cover short-term medications such as antibiotics.

In addition to free and low cost medications for chronic conditions, several area pharmacies offer some prescription medicines at a lower cost. MyGULFCare - Care Management and Health Coaching

Care Management: MyGulfCare's Care Management Programis for people with a chronic health condition who need personalized, one-on-one assistance to understand their condition and how to control it. Care Management currently is offered to individuals with one or more of the following: diabetes, high blood pressure, or high cholesterol.

MyGulfCare's Care Manager will meet with you one-on-one to discuss your present health condition and help you set short and long-term health goals. The Care Manager will provide you with the tools you need to better control your chronic condition and will work with you to make sure you are making progress.

Health Coaching: If you are interested in making healthy lifestyle changes such as weight loss, increased physical activity, stress management, healthy eating, or smoking cessation, then MyGulfCare's Health Coaching Program can help you get started and provide you with the tools and support you need to be successful.

Our health coaching program offers assistance in lifestyle changes, such as weight management, stress reduction, and fitness plans. The Health Coaching Program involves one-to-one K.N.E.W. (Knowledge, Nutrition, Exercise, Wellness) Plans, as well as optional group and community activities. This program requires physician approval, which we can help you obtain.
