The first six weeks focus on overall healthy living, with two additional sessions that cover kidney specific content. Participants will discuss quality of life with kidney disease, have a panel discussion with healthcare professionals about dialysis and transplant, and discuss getting the most out of their kidney care. A key activity is development of a kidney disease "toolkit" which is not a physical toolkit, but a set of strategies that people can use to help manage their kidney disease.
