It is estimated that between 1.6 and 3.8 million concussions occur in the USA per year during competitive sports and recreational activities; however, as many as 50% of concussions may go unreported*.
Ascension Medical Group physicians and licensed athletic trainers are concerned about the current treatment options available to club, middle school and local athletic programs. As a result, Ascension Medical Group has partnered with ImPACT® in an effort to provide state-of-the-art concussion care to athletes throughout the Fox Valley.
This program offers a Baseline ImPACT® test for any local athlete who does not already have access to a cognitive test through their club or school. These tests are to be done before a concussion occurs in order to establish a baseline “or normal” cognitive score.
ImPACT: What is it?
ImPACT's Test is computerized and takes about 25 minutes to complete. ImPACT recommends that it be administered by an ImPACT trained athletic trainer, team doctor or psychologist. Baseline tests are suggested every two years. If a concussion is suspected, the baseline report will serve as a comparison to a repeat ImPACT test, which professionals can use to assess potential changes or damage caused by a concussion^.

If a concussion is suspected, please contact an Ascension provider who is clinically trained in concussion management and ImPACT. A complete list can be found by contacting Nurse Direct at 1-800-362-9900.
Time: 6:30 pm
Testing takes approximately 25 minutes. Please plan on appointment taking up to 60 minutes for proper instruction and directions.
Costs: $10 per Baseline test

St. Elizabeth's Hospital, Appleton, WI
Mercy Medical Center, Oshkosh, WI
Frequency: Starting at the age of 11 years old, Baseline tests are valid for two years (must be 11 years of age at the time of the test)
What happens to my results?
The results of the baseline are stored in the Affinity Health System secured server, which is maintained by ImPACT®. Test results are accessed after a concussion occurs; at which time, the baseline results are compared to a post-injury test. You will need your Passport ID for physician to access the results of the baseline test. Post-injury tests are to be scheduled separately at an Affinity concussion provider clinic.
How do I access my results?
Hopefully you will never need to access your results. Baseline results are accessed only after a concussion occurs. If your son/daughter sustains a concussion after taking their Baseline test; an appointment should be made with a specially trained Affinity concussion physician. The results will be available at the appointment and reviewed by the physician.
