St. Elizabeth Hospital Foundation was founded in 1976 and serves as the philanthropic arm of St. Elizabeth Hospital, Calumet Medical Center and Affinity Medical Group clinics located throughout Outagamie, Calumet and northern Winnebago Counties.

Mission
The foundation's primary purpose is to live out Affinity Health System's Mission, Promise and Values. Our foundation takes a special interest in serving the needs of the poor; we invite you to help make a difference in the lives of those who have nowhere else to turn. The mission of the St. Elizabeth Hospital Foundation is to live out the healing ministry of Christ with a special emphasis on those in need, by generating, stewarding and distributing funds to enhance the quality of the health system's services and programs, and other community health initiatives.

Your Support Matters
For over 40 years, we have worked with you, our donors and community. Thank you for your support and investment in serving our important mission. Our 2017 Annual Report provides information on the many life saving programs and initiatives that wouldn't happen without you and your generous support.
