Transitional Care

Transitional care is available for those individuals who require a few extra days or weeks of nursing and therapies following a hospital stay or an illness. Extended or long-term care is available for those who need care for longer periods of time.

Geriatric nurse specialists and the Hospital Elder Life Program (HELP) can assist geriatric patients with dementia.
