We are currently offering COVID-19 vaccination appointments for patients age 75 years or older, frontline essential workers, persons working or living in congregate settings including but not limited to homeless shelters and group homes, and first responders including firefighters and law enforcement. Once you are eligible to receive a vaccine, you will receive an e-mail from your doctor at Ascension Providence. We are working with local and state government and health agencies to help distribute vaccinations as they become available.

    We are providing the COVID-19 vaccine to community members who are age 65 and older. Appointments for the vaccine must be scheduled in advance. Persons without an appointment will not be able to receive the vaccine and walk-ins will not be accepted.     Ascension Sacred Heart Emerald Coast - Miramar Beach

    We have begun providing the COVID-19 vaccine to our patients who are age 65 and older. Once you are eligible to schedule a vaccine, you will receive information from your doctor at Ascension Sacred Heart. Appointments for the vaccine must be scheduled in advance. Persons without an appointment will not be able to receive the vaccine and walk-ins will not be accepted.     Ascension Sacred Heart Gulf - Port St. Joe

    Working with the Florida Department of Health in Gulf County, we are providing the COVID-19 vaccine to community members who are age 65 and older. Appointments for the vaccine must be scheduled in advance through the health department at http://gulf.floridahealth.gov/newsroom/2021/01/vaccine-registration.html. Persons without an appointment will not be able to receive the vaccine and walk-ins will not be accepted.

  • In accordance with state and local guidance, Ascension Michigan is scheduling appointments for administering the COVID-19 vaccine to our high-risk Ascension Medical Group patients who are age 65 and older. These vaccines will be provided at our regional community vaccination clinic sites across the state.

    We are proactively and personally contacting eligible patients by email, text and phone to schedule their appointment. Vaccinations must be scheduled in advance, and walk-ins will not be accepted. Persons without an appointment will not be eligible for vaccination at this time.

    Ascension Michigan serves approximately 150,000 Ascension Medical Group patients who are age 65 and above. As vaccine supply remains limited, it will take several weeks to successfully schedule and administer vaccines to this patient population.

    In addition to vaccinating our Ascension Medical Group eligible patients, we are partnering with the county health departments to assist with vaccinating area healthcare professionals and qualified essential workers per current state of Michigan guidelines.

    In the meantime, the Michigan Department of Health and Human Services, your local public health authority and/or the Centers for Disease Prevention and Control (CDC) website can provide you the latest information regarding vaccine distribution.

    We appreciate everyone’s patience and understanding as we continue to support the State of Michigan in this statewide vaccination effort.     To our valued patients and community members

    As we vaccinate to help protect your family and ours from COVID-19, we are eager to reach as many people as possible to achieve a point of community immunity. We will be making the vaccination available to you, our AMG patients, in coordination with state and local guidance.

    We’ll keep you updated when you are eligible for the vaccine

    Consistent with guidance from our state and local government and health agencies, we are currently only offering the vaccine to our established Ascension Medical Group patients who are 75 years and older, with known high risk conditions.

    Though the vaccine is in limited supply now, we expect it to be more widely available across our communities in the coming weeks and months. As we receive more supply, we will expand to additional members of our AMG Saint Thomas patient population. When it is time for you to receive your vaccine, we will notify you via the email and/or text number that we have on file. We are grateful for the overwhelmingly positive response as we work to administer available vaccines.

    Ascension Medical Group (AMG) Seton, the network of our employed provider practice groups associated with Ascension Seton, is providing COVID-19 vaccinations to high-risk AMG primary care patients in accordance with Texas Department of State Health Services guidelines. We are currently focused on Phase 1B patient population, including AMG patients who are 65 years of age or older; or patients who are 16 or over with chronic conditions such as cancer, chronic kidney disease, heart conditions, solid organ transplantation, obesity, pregnancy, and sickle cell disease. AMG is reviewing medical records for all existing primary care patients and contacting eligible patients by email, text and/or phone to schedule vaccine appointments. Walk-ins are not accepted. Please do not call our hospitals or clinics regarding vaccinations. It will take time to contact all eligible patients, and appointment availability is determined by our vaccine allocation from the state. Vaccines are still limited at this time.   • Ascension Medical Group (AMG) Wisconsin is now scheduling COVID-19 vaccinations to high-risk AMG patients in accordance with Wisconsin Department Health Services guidelines. We are currently focused on the patient population identified by the Wisconsin Department Health Services in Phase 1B. This includes AMG patients who are 65 years of age or older as well as frontline health care workers, residents in long term care facilities (nursing homes and assisted living facilities), and police and fire personnel.

    AMG is in the process of reviewing medical records for all existing patients and contacting all patients who fit the above criteria within Phase 1B to schedule vaccine appointments. Please do not call our hospitals or clinics regarding vaccinations, as vaccines are limited at this time, and AMG will reach out directly to patients who are eligible for the vaccine. If Ascension Wisconsin continues to receive additional doses of the COVID-19 vaccine, we will continue to expand vaccine operations consistent with federal and state guidance. We will share more details publicly when we move to the next phase of vaccination for the larger phase 1B group.
